Our office is holding enrollment fees as our way of helping during these unprecedented times.
At Envision Health, we strive to provide accessible, affordable primary care to patients that value health, a relationship with their doctor, and playing an active role in the decision-making process.
It's called Direct Primary Care.
How does it work? Members of the practice pay a monthly fee (no insurance is billed). Eliminating insurance allows the physician to devote time to the patient instead of working on non-medical tasks. The membership includes office visits, access to discounted prescription medications, and a whole lot more. Instead of the 2,000-3,000 patients in a traditional practice, this setting limits the number of patients to approximately 300. This allows the physician to know and understand the needs of each patient as an individual.

We're a Direct Primary Care (DPC) practice. DPC is a unique approach to healthcare and our patients love it. So, how is it different?
Did you know that most Primary Care Providers have between 2,500-3,500 patients? We limit our practice to under 300 patients total. That means that your doctor spends more time with you when you need her. Plus, when she is not actively seeing a patient, she is able to help coordinate your care and research your treatments.
Envision Health does not have to deal with any insurance plans so you won’t have a "middle-man" in your primary care. Decisions about your healthcare will be made by you and your doctor. The only loyalty your doctor has is to you. Instead, patients pay a monthly membership fee which can save money and covers excellent treatment. Give it a try, and you’ll quickly see for yourself the value of this approach!
No need to wait weeks for an appointment. Same day or next business day visits are typically available and included in your membership.
Calls and messages will be answered directly by your physician. Better yet, come in for a face to face visit.
We’ve negotiated rates on labs, radiology, and some commonly prescribed medications. Access to these savings are included in the membership.
Dr. Cathleen Mok is a board certified osteopathic family physician, who grew up in Leominster, Massachusetts. After graduating with a bachelor's degree in Chemistry from Boston University, she attended Midwestern University Chicago College of Osteopathic Medicine. While in medical school, she completed an undergraduate fellowship in Osteopathic Manipulative Medicine. After graduation from medical school, she completed a rotating internship at Illinois Masonic Medical Center, followed by a Family Medicine residency at LaGrange Hospital. Upon completion of residency, she began her career in a "traditional" Family Medicine practice, where she enjoyed caring for her patients and their families. As her practice grew, so did her family. She had to choose between the demands of a busy practice and needs of a growing family. In order to meet the needs of her family, she transitioned into the field of Occupational Medicine. While different than her true passion of primary care, Occupational Medicine allowed Dr. Mok to learn a new specialty, build upon her procedural skills, and shape her acupuncture practice. In the meanwhile, she continued expanding her knowledge about healthy eating and maintaining an active lifestyle to promote health and prevent disease. While she was able to maintain her professional growth, she realized that her true passion was still in primary care. Unfortunately, she realized that the "traditional" model of healthcare did not align with her view of a physician-patient relationship. In 2019, she learned of a "new" way to deliver healthcare. . . Direct Primary Care.
